What types of insurance does your office take?
Family & Industrial Medical Center accepts most major PPO Insurance Companies such as: Blue Cross, Blue Shield (including Covered California), Aetna, Cigna, United Health Care, Health Net, TriCare, and Medicare. We do not accept Medi-Cal, Cencal, nor Physician’s Choice plans as primary or secondary insurances. Medi-Cal and Cencal policy holders cannot self-pay.
If you are unsure if we are a covered provider with your insurance, please call your insurance company and let them know our name and address and they will be able to provide you with that information. All visits are financially the patient’s responsibility. It is your responsibility to know if your insurance will cover your visit.

What if I have an HMO plan?
We are members of Coastal Communities Physicians Network (CCPN) for our HMO patients. Please call CCPN to select us as your Primary Care Physician (PCP). If you are assigned to another PCP or HMO network, you may still take advantage of our Urgent Care services for your acute needs.
Do you have an HMO and are visiting from out of the area? Out of network coverage may be available through your insurance plan! Please call the customer service number on the back of your insurance card to find out if they cover you or your family through an out of network Urgent Care provider. Family & Industrial Medical Center is recognized by most insurance carriers as an Urgent Care Facility.
How do I make an appointment?

Do you accept walk-ins?
Yes, since we are an Urgent Care center, feel free to walk-in for any minor traumas/illnesses. You can also make a same-day urgent care reservation at urgentcareslo.com. Just don’t forget to establish with one of our providers if you will be visiting us on a more regular basis!
